HIPAA Form

One of our roles as the McGohan Brabender Customer Care Team is to act as your advocate to carriers and providers, ensuring that benefit plans operate properly. Our knowledge of healthcare as well as our relationship with carriers allows our team to troubleshoot when issues arise, and our commitment to providing the best customer service experience to help members resolve their claims. We always are happy to work directly with the member to resolve claim issues; however, if the person would prefer that we work through someone other than the employee, we require a HIPAA release form.

You may remember the last time you went to your health care providers they asked you to complete a HIPAA form.  That form asked you to list the people who the provider could release your medical information to. This form is required by the government so that your health information will be protected and remain confidential.  Just like your health care provider, we are required to have a HIPAA release forms signed by the member before we can release the member’s medical information to someone other than the employee.
